CommuniCare serves families across New York City's five boroughs. Services are thoughtfully matched to each child's individual needs, with availability varying by discipline, location, and language.
Early Intervention Referrals
To begin the Early Intervention process in New York City, families can call 311 and ask for Early Intervention. A service coordinator will help guide families through the evaluation process. If a child is found eligible, the family and Early Intervention team work together to develop an Individualized Family Service Plan (IFSP) based on the child's needs and the family's priorities. Parents can refer their child directly to Early Intervention without a doctor's referral.
